Who might be able to join this trial:
- People who have moderate-to-moderately-severe chronic kidney disease (CKD Stages 3 or 4), people who have received a kidney transplant with any level of kidney function, or people without kidney disease who are participating as comparison subjects
- People who are not currently active — meaning they exercise less than 20 minutes, twice per week
- CKD patients whose kidney function has been stable (not declining significantly) over the past 6 months, and whose blood acid-base levels fall within a specific normal range
- People who also have high blood pressure (hypertension)
Who may not be able to join:
- People with very severe kidney disease (kidney function below a certain threshold)
- People with a blood chemistry imbalance where the blood is too alkaline (confirm with trial site)
- People currently taking bicarbonate supplements or medication
- People with ongoing drug or alcohol misuse
- People with nerve damage related to diabetes, or problems with the body's automatic nervous system
- People with a serious illness that could affect life expectancy
- People with anaemia where their haemoglobin (a measure of red blood cells) is below a certain level
- People with clinical signs of heart failure
- People with excess fluid in the body or reduced heart pumping strength below a certain level
- People with heart disease symptoms identified through heart tests or medical history
- People being treated with a blood pressure medication called clonidine (a central alpha-agonist)
- People who have had a heart attack or stroke within the past six months
- People with poorly controlled high blood pressure above a certain level
- People with blood pressure that is too low
- People who have had surgery within the past 3 months
- People who are pregnant or planning to become pregnant
- People who are unable to ride a stationary exercise bicycle
- People for whom it would not be safe to temporarily stop certain blood pressure medications (alpha- and beta-blockers)
- People with disease affecting the arteries in the legs or arms (peripheral arterial disease)
- People with a BMI over 40 (classified as class 3 obesity)
- People with potassium levels in the blood that are too low or too high
- People currently taking immune-suppressing medications, such as steroids, certain transplant medications, or similar drugs — note: kidney transplant recipients who are on immunosuppression would not be eligible (confirm with trial site)
- People who have a dialysis access point in their body, such as an AV fistula or graft
- People who cannot safely have an MRI scan, including those with a pacemaker, cochlear implants, nerve stimulators, implanted metal devices, other metal in the body, or a history of claustrophobia
