Who might be able to join this trial:

  • Adults aged 18 or older, of any sex
  • People whose chronic kidney disease (CKD) diagnosis timing matches the study's requirements
  • People with CKD who are not currently on dialysis, with kidney function falling within a specific moderate range (Stage 2 to 3b, as measured by a kidney filtering test called GFR)
  • People whose urine test shows a certain level of protein leaking into the urine (albumin-to-creatinine ratio of 30 mg/g or higher)
  • People whose blood potassium level is below a certain threshold (under 5 mEq/L)
  • People who have recently been diagnosed with high blood pressure

Who may not be able to join:

  • People with a high potassium level in their blood (5 mEq/L or above)
  • People with diabetes
  • People with cancer
  • People with heart disease
  • People with liver or bile duct disease, or any other liver condition
  • People with kidney stones or a urinary tract infection
  • People with an overactive thyroid gland
  • People with a bleeding disorder
  • People who have had an allergic reaction to a class of blood pressure medicines known as ACE inhibitors or ARBs
  • Women who are pregnant or breastfeeding
  • People whose blood pressure is very high (180/110 or above) or very low (below 100/60)
  • People currently taking certain medications that could interact with the study drug ramipril — including alteplase, azathioprine, everolimus, sirolimus, lithium, anti-inflammatory pain medicines (such as diclofenac, tenoxicam, or celecoxib), or water tablets that retain potassium (such as amiloride or spironolactone), or any other ACE inhibitor or ARB medicines
  • People currently taking certain supplements or medicines that could interact with another study substance (Coenzyme Q10) — including omega-3 fatty acids, vitamins A, C, E, or K, chemotherapy drugs, the blood thinner warfarin, cholestyramine, or orlistat

Primary endpoints

The change in kidney function test measured by creatinine clearance (eGFR) mL/min/1.73m2 which will be calculated using the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) creatinine equation, 2021; The change in proteinuria level be assessed using Albumin-to-creatinine ratio (ACR) ratio (mg/g); The change in blood pressure (mmHg) will be done using a mercury sphygmomanometer; The change in Blood urea nitrogen (BUN) (mg/dl); The change in serum potassium (meq/l).; The change in serum creatinine (mg/dl); The change in serum urea (mg/dl)
