Who might be able to join this trial:
- You are between 40 and 70 years old.
- You are willing to give your agreement (informed consent) to take part in the study.
- A blood test taken in the last six months showed your uric acid level was above 5 mg/dl.
- You meet at least one of the following signs of high or very high heart and blood vessel risk:
- A calculation of your 10-year risk of dying from a heart or blood vessel problem is above 2.5% (if you are under 50) or 5% or more (if you are 50 or older), based on a specific risk scoring tool (confirm with trial site)
- You have been diagnosed with a heart or blood vessel condition such as a stroke, mini-stroke (TIA), brain bleed, mild-to-moderate heart failure (not caused by blocked heart arteries), peripheral artery disease, or atrial fibrillation
- You have diabetes or high blood pressure that has caused damage to organs, shown by any of the following: stiff arteries, thickening of the heart wall, a small amount of protein leaking into your urine, or reduced blood flow to your legs (confirm with trial site)
Who may not be able to join:
- You are currently taking allopurinol, febuxostat, or any other medication to lower uric acid levels.
- You have a medical reason why you cannot take allopurinol.
- You are pregnant, breastfeeding, or planning to become pregnant during the study period.
- You are currently taking hormone therapy that contains oestrogens.
- You have had an active cancer diagnosis in the last five years (with some exceptions for certain localised tumours — confirm with trial site).
- Your blood pressure is very poorly controlled (average reading of 180/110 mmHg or higher in home measurements in the week before screening), even while taking blood pressure medication.
- Your kidneys are not working well enough, based on a specific kidney function measurement falling below a certain level (confirm with trial site).
- You have an underactive or overactive thyroid that is not currently well managed with treatment.
- You have confirmed coronary artery disease, such as a previous heart attack, a procedure to open blocked heart arteries, or confirmed artery blockages found on a scan.
- You have severe heart failure (classified as NYHA class III or IV — confirm with trial site).
- You are currently taking azathioprine, mercaptopurine, or cyclosporin.
- You have taken part in another clinical trial of a medicine or medical device within the last three months (or longer, depending on the medication involved — confirm with trial site).
