Who might be able to join this trial:
- Adults aged 18 or older
- People who receive primary care at one of three Harris Health clinics: Smith Clinic, Harris Health Outpatient Center (on the LBJ campus), or Martin Luther King Jr. Health Center (MLK)
- People who have had at least one visit at the clinic in the past 18 months (not a first-time patient)
- People diagnosed with early-to-moderate chronic kidney disease (CKD Stage 2, 3a, or 3b), or early/undiagnosed kidney disease with certain protein levels in the urine (confirm with trial site)
- People whose most recent blood potassium level, tested within the past 12 months, was at or below a certain level (confirm with trial site)
- People who speak English or Spanish
- People who are able to participate in the program for at least 6 months
- People who are able to clean, prepare, and store (refrigerate or freeze) food products provided through the trial
- People who have access to receive text messages (SMS) on a mobile phone
- People whose preferred delivery address falls within an available delivery zone
Who may not be able to join:
- People with more advanced kidney disease (CKD Stage 4), end-stage kidney disease, or who are currently on dialysis
- People who regularly take certain pain medications called NSAIDs (such as ibuprofen) more than twice a week for 90 days or longer, as these can affect potassium levels
- People who take a type of medication called mineralocorticoid receptor antagonists (confirm with trial site for examples)
- People who take the blood-thinning medication Warfarin
- People with certain specific kidney conditions — such as polycystic kidney disease, glomerulonephritis, lupus-related kidney disease, or a condition called ANCA-associated vasculitis — where the treating doctor determines participation would not be appropriate
- People with a history of an organ transplant, where the treating doctor determines participation would not be appropriate
- People who have received immune system treatment for a kidney condition within the 6 months before enrolling
- People with severe heart failure classified as Class IV by their doctor
- People who have had a heart attack, unstable chest pain, stroke, or mini-stroke (TIA) within the 12 weeks before enrolling
- People who have had a heart procedure (such as stenting, bypass surgery, or heart valve repair) within the 12 weeks before enrolling
- People who are currently receiving hospice care
- People with an active cancer requiring treatment, where the treating doctor determines participation would not be appropriate
- People with severe liver disease (decompensated cirrhosis), as determined by their doctor
- People with cognitive (thinking or memory) difficulties that would prevent participation, as identified during enrollment screening
