Who might be able to join this trial:
- People aged 55 or older who have a primary care doctor at Brigham (or an appointment at one of 15 specific locations) and are being discharged from a Brigham and Women's Hospital general medicine unit
- People being discharged to their own home, a home health care service, or an assisted living facility
- People (or their designated healthcare decision-maker) who can speak English fluently
- People who have at least one of the following conditions listed in their medical record — heart failure, type 2 diabetes, or chronic kidney disease — plus at least one other ongoing health condition also listed in their medical record
Who may not be able to join:
- People aged 55 or older whose primary care doctor is based at the Westwood, Pembroke, or Transition Clinic locations and who were admitted to an ICU, or an obstetrics, surgical, cardiology, oncology, orthopaedics, or other specialist unit
- People who are currently pregnant
- People who are a prisoner, held in a detention facility, or in police custody
- People whose discharge from hospital is planned within 3 hours of being assessed for the trial
- People who are too unwell to take part, or who have active psychosis, serious mental illness, delirium, or severe dementia
- People (and whose healthcare decision-maker) cannot speak English fluently
- People who are not expected to be discharged to a home setting
- People who do not have a device capable of running the trial's app (confirm with trial site)
- People who do not have a working telephone available for a 30-day follow-up call
