Who might be able to join this trial:
- Adults aged 18 or older who have end-stage kidney disease (kidneys that have stopped working) and have been assessed as pre-frail, frail, or very frail using a standard frailty measurement tool called Fried's Frailty Phenotype
- People who are on, or in the process of being added to, the kidney transplant waiting list at one of three participating hospitals (McGill University Health Centre, Centre Hospitalier de l'Université de Montréal, or University of Alberta Hospital), whether for a first transplant or a repeat transplant
- People who speak English or French
- People who are able to join video calls online, either on their own or with help from someone in their household — people without internet access may be able to borrow a tablet for the duration of the study (confirm with trial site)
Who may not be able to join:
- People who score as "robust" (not frail) on the Fried's Frailty Phenotype assessment
- People who are already taking part in a structured exercise program, whether at a hospital, at home, or through another research trial
- People who are waiting for a combined kidney-pancreas or kidney-liver transplant, as those waiting lists tend to be much longer
- People who are currently in hospital for any reason at the time eligibility is being assessed
- People with a very high level of antibody sensitisation (above 95% cPRA) who are on a special exchange program and expected to wait a long time for a transplant (confirm with trial site)
- People who are expected to receive a transplant before the 12-week program is finished — for example, those receiving a kidney from a living donor or those with type A blood
- People who have significant cognitive impairment (serious difficulties with memory or thinking), whether pre-existing or newly identified
- People who have a pre-existing or newly identified heart condition, bone or joint condition, nerve condition, or neurological condition that could affect their ability to exercise safely
