Who might be able to join this trial:
- You identify as African American and are a military veteran
- You are older than 18 years of age
- Your kidney function test results fall within a specific moderate range, showing either moderately reduced kidney function with or without protein in the urine, or mildly reduced kidney function with a notable amount of protein in the urine (confirm with trial site)

- You have been diagnosed with Type 1 or Type 2 Diabetes

- You take a cholesterol-lowering medication (statin) and have not been on the same stable dose for at least one month
- You have a confirmed kidney disease caused by damage to the kidney's filtering units, proven by a tissue sample (biopsy)
- You are currently an active smoker
- You currently have open skin wounds being treated, or have had surgery within the last month
- You are pregnant, planning to become pregnant, currently breastfeeding, or are able to become pregnant but not using birth control
- You have had an allergic reaction, including severe swelling, to the study drug empagliflozin (Jardiance) or any of its ingredients
- You are currently on dialysis (kidney replacement therapy)
- Your kidney function is severely reduced below the minimum level required for this trial (confirm with trial site)
- You have planned surgery or a planned hospital stay within 5 months of joining the study
- You have a history of heart failure with a weakened pumping function, confirmed by a specific heart test result (confirm with trial site)
- You have been diagnosed with Polycystic Kidney Disease (a genetic condition causing cysts on the kidneys)
