Patients:

Patients:
- People currently receiving hospice care.
- People currently receiving dialysis.
- People whose medical records show active severe mental illness that is not being treated with medication, moderate-to-severe dementia, thoughts of suicide, uncorrected hearing loss, or active substance abuse.
Caregivers:
- People who report active mental illness that is not being treated with medication, moderate-to-severe dementia, active thoughts of suicide, uncorrected hearing loss, or active substance abuse (as identified through standard study screening questions).
