Who might be able to join this trial:
- People with a BMI of 40 or higher at the time they are admitted to hospital to have their labour started
- People who speak English or Spanish
- People who are between 34 weeks and 42 weeks and 6 days pregnant
- People aged 18 years or older
- People carrying a single baby who is alive and positioned head-down
- People whose cervix (the opening of the womb) is dilated less than 5 cm and who are planning to have cervical ripening (a process used to prepare the cervix before labour)
- People having fewer than 5 contractions every 10 minutes
Who may not be able to join:
- People who have previously had a caesarean section (C-section)
- People for whom prostaglandin medication (used to ripen the cervix) is not medically appropriate — for example, those who have had significant surgery on the uterus or a prior caesarean delivery
- People for whom vaginal birth is not medically appropriate — for example, those with placenta previa, vasa previa, or HIV with a high viral load
- People for whom going into labour is not medically appropriate — for example, those with certain heart conditions, brain or nerve surgery needs, or a medical need for a caesarean
- People under 18 years of age
- People whose baby has restricted growth along with abnormal blood flow readings in the umbilical cord
- People whose cervix is dilated more than 5 cm
- People having more than 5 contractions every 10 minutes
- People with significant vaginal bleeding where there is concern about the placenta separating early (placental abruption)
- People whose baby is showing signs of distress or abnormal heart rate patterns
- People whose baby has not survived or has a major birth defect
- People who are unable to give informed consent
