• People with moderate to severe cognitive impairment, such as a dementia diagnosis or a low score on a cognitive screening test
• People who need an interpreter for their medical appointments
• People with a severe psychiatric condition, including a psychiatric hospital stay in the past year, or a diagnosis of substance use disorder, bipolar disorder, or psychosis
• People who are currently enrolled in hospice care
• People whose oncology doctor feels they would be unable to complete the study activities, for example due to significant fatigue
