Who might be able to join this trial:

• People aged 22 or older in the USA, or 18 or older outside the USA.
• People who have been diagnosed with stage 4 non-small cell lung cancer (NSCLC) — either the squamous or non-squamous type — confirmed by laboratory testing of tissue or cells.
• People whose lung cancer can be assessed or measured using standard imaging methods in the chest area.
• People who have not yet received any systemic (whole-body) treatment for their stage 4 lung cancer. People who previously received chemotherapy or chemo-radiation treatment aimed at curing an earlier, non-metastatic stage of the disease may still be considered, provided that treatment finished at least 12 months before the cancer spread to other parts of the body.
• People with a good general health and activity level, rated 0 or 1 on a standard medical scale (meaning fully active or able to carry out light work).
• People with adequate blood counts and organ function as determined by lab tests.
• Women who are not pregnant and not breastfeeding at the time of joining.
• Men with a female partner who could become pregnant must agree to use effective contraception during the trial.
• All participants must be willing and able to sign a written consent form.

Who may not be able to join:

• People whose lung cancer is a mixed type involving both small cell and non-small cell components.
• People whose cancer has certain specific genetic changes (such as EGFR, ALK, ROS1, RET, METex14, NTRK1/2, or BRAF V600 mutations) where targeted treatments are locally approved and available and are planned or recommended for their care.
• People who have already received systemic treatment for their metastatic (spread) lung cancer.
• People who had major surgery less than 3 weeks before entering the trial.
• People who received a high dose of radiation (more than 30 Gy) to the lungs within the 6 months before joining.
• People who received any radiotherapy within 2 weeks before joining, unless it was a short course of palliative (symptom-relieving) radiation to an area outside the brain, in which case a 1-week gap may be acceptable (confirm with trial site). Participants must also have recovered from any radiation side effects and not need steroid medications as a result.
• People who are expected to need other cancer treatments during the trial.
• People who have another active cancer that is growing or has needed treatment within the past 3 years. (Note: certain skin cancers and some in-situ cancers that have been treated with curative intent are not automatically excluded — confirm with trial site.)
• People with active, symptomatic brain or spinal fluid involvement from their cancer. People with brain metastases that have already been treated and are stable, without the need for steroid medications for at least 7 days, may still be considered.
• People with an active autoimmune disease that has needed systemic medical treatment within the past 2 years.
• People with an immune system disorder, or who are currently taking immune-suppressing medications or steroid medications above a certain dose within 7 days before joining. People with asthma using inhalers or occasional steroid injections are not automatically excluded.
• People who have previously received certain types of immunotherapy drugs (anti-PD-1, PD-L1, or PD-L2 agents, or similar immune-targeting treatments) within the 12 months before joining.
• People who have taken part in another clinical trial involving an experimental treatment or device within the 4 weeks before joining.
• People receiving other experimental NSCLC treatments at the same time as this trial.
• People with a known allergy or sensitivity to any of the medicines used in this trial (including pembrolizumab, cisplatin, carboplatin, pemetrexed, paclitaxel, or nab-paclitaxel).
• People who are pregnant or breastfeeding.
• People who are in an institution under an administrative or court order.
