Who might be able to join this trial:

• People who have been diagnosed with liver cancer (hepatocellular carcinoma), either for the first time or as a recurrence.
• People whose liver cancer has spread to the lungs either within one month of the liver cancer diagnosis or more than one month after.
• People who have more than 5 but no more than 10 spots of cancer spread in the lungs only.
• People whose lung metastases (cancer spots in the lungs) are each no larger than 5 cm in size.
• People who have 5 or fewer tumours in the liver, and where the total tumour size takes up no more than half the liver.
• People whose cancer involvement in the main liver blood vessel (portal vein) is at a level classified as Vp3 or lower (confirm with trial site).
• People whose first round of standard drug treatment — such as Sorafenib or Lenvatinib, with or without certain immunotherapy drugs — has stopped working.
• People whose liver tumours have been responding to treatment (either shrinking or staying stable) and whose lung spots have not been getting worse, as measured by a standard medical assessment tool called mRECIST.
• People who have also received local liver treatments such as TACE or HAIC (procedures that deliver treatment directly to liver tumours).
• People whose liver function is rated as Child-Pugh class A or B, which are measures of how well the liver is working (confirm with trial site).
• People with a general health performance score of 0 or 1, meaning they are fully active or have only minor limitations in daily activity.
• People with no history of any other type of cancer.

Who may not be able to join:

• People younger than 18 years old or older than 75 years old.
• People who have more than 10 spots of cancer spread in the lungs.
• People whose cancer has spread to parts of the body other than the lungs.
• People whose medical records are incomplete.
• People whose lung cancer spots are larger than 5 cm in size.
• People who have more than 5 tumours in the liver, or where the tumours take up more than half the liver volume.
• People whose cancer involvement in the main liver blood vessel is at the most severe level, classified as Vp4 (confirm with trial site).
• People who were lost to follow-up (meaning contact with the medical team was lost) within the first 3 months.
