  • Adults aged 18 or older who have been diagnosed with chronic kidney disease (CKD) and have a greater than 10% chance of needing dialysis within 2 years, based on a specific kidney risk calculation tool
  • People who are registered as patients at a kidney specialist (nephrology) clinic, including a dialysis clinic, at one of the participating hospitals
  • People whose kidney disease has been confirmed by their kidney specialist as stable for at least 1 month before joining the trial

Who may not be able to join:

  • People who have previously received a kidney transplant, or who are already on a waiting list for an organ transplant at the time of joining
  • People with serious health conditions that would prevent full participation in the trial, such as dementia or clinically severe depression
  • People who have been diagnosed with a medical condition where life expectancy is estimated to be less than 2 years
