For patients:

- People who have been diagnosed with Alzheimer's disease or a related form of dementia (including vascular dementia, Lewy body dementia, frontotemporal dementia, or dementia of unknown type)

- People who are able to give their own informed consent to participate
For oncology clinicians:
- Cancer specialists who are affiliated with the Wilmot Cancer Institute
- Clinicians who do not plan to move or retire within the next two years
