Who might be able to join this trial:
- Adults who receive care at Johns Hopkins Medicine and live in Baltimore City, or adults who receive care at Johns Hopkins Community Physicians Hagerstown or Family Healthcare of Hagerstown and live in Washington County
- People living in an area ranked above the 75th percentile on a Maryland socioeconomic disadvantage index, combined with having a low income
- People who have been diagnosed with type 2 diabetes
- People with a body mass index (BMI) of 30 or higher, which is the medical threshold for obesity
Who may not be able to join:
- People younger than 30 or older than 70 years of age
- People who already have heart failure
- People whose blood sugar levels are poorly controlled (too low, very high, or whose most recent HbA1c blood test result was 11% or above)
- People with high blood pressure that is not under control, whether or not they are taking medication for it
- People with a known blockage in the heart's arteries, unless imaging has shown the blockage is less than 50% (confirm with trial site)
- People with moderate or severe disease affecting the heart valves
- People with serious medical conditions that affect life expectancy or that currently require active medical management
- People whose responses to a standard physical activity readiness questionnaire suggest they are unable to take part in moderate-intensity physical activity
- People with any condition, or a planned surgery or procedure, that would prevent them from doing 150 or more minutes of exercise per week
- People with end-stage kidney disease
- People currently taking part in another behaviour change program
- People with an active alcohol or substance use disorder
- People already doing more than 60 minutes per week of moderate to vigorous exercise on a regular basis
- People who are currently pregnant
- People whose baseline exercise stress test shows signs of reduced blood flow to the heart, a dangerous heart rhythm, or other signs of clinical instability
