Who might be able to join this trial:

• You are willing to take part voluntarily and have signed a consent form approved by an ethics review board
• You are willing and able to follow all study procedures for the full length of the study
• You are male or female
• You are 18 years of age or older
• You have been diagnosed with advanced or metastatic non-small cell lung cancer that has continued to grow or spread after at least 2 previous treatments (any type or subtype)
• You have been diagnosed with extensive stage small cell lung cancer that has continued to grow or spread after at least 2 previous treatments
• You are able to swallow pills and are willing to follow the medication schedule for the study drug (fingolimod)
• You are generally able to care for yourself and carry out some daily activities (confirm with trial site)
• Your blood test shows a lymphocyte (a type of white blood cell) count above 1,000 cells/mL
• If you are a woman who could become pregnant: you have been using highly effective birth control for at least 1 month before screening and agree to continue during the study and for 2 months after finishing the study drug
• If you are a man who could father a child: you agree to use condoms or other contraception during the study and for 2 months after finishing the study drug

Who may not be able to join:

• You have had a serious heart event in the last 6 months, such as a heart attack, stroke, mini-stroke, unstable chest pain, severe heart failure requiring a hospital stay, or you have a condition called congenital long QT syndrome
• You are taking certain medications that interact significantly with the study drug and cannot safely be switched to something else before starting the trial (a pharmacist and the study doctors will review your medications)
• You are taking medications that are known to cause a specific dangerous heart rhythm problem (called torsade de pointes) (confirm with trial site)
• You are taking medications that slow your heart rate or affect how electrical signals travel through your heart
• You are currently receiving other cancer treatments, immune-suppressing drugs, or immune-modifying therapies
• You are taking a medication called ketoconazole (an antifungal) and have not stopped taking it at least 2 weeks before starting the study
• You have brain metastases (cancer that has spread to the brain) that have not been treated, or that are causing symptoms — you may still be eligible if your brain metastases were previously treated, you have no symptoms, and your condition has been stable for at least 2 weeks
• You have a history of certain heart rhythm or electrical conduction problems (such as specific types of heart block or sick sinus syndrome), unless you have a working pacemaker
• A specific measurement of your heart's electrical activity (called the QTc interval) is 500 milliseconds or more on your heart tracing (ECG)
• You need to take certain specific heart rhythm medications, including disopyramide, quinidine, procainamide, dronedarone, dofetilide, sotalol, ibutilide, or amiodarone
• You have had an allergic reaction (such as a rash, hives, or swelling) to the study drug fingolimod or any of its ingredients
• You currently have an active infection (bacterial, viral, or fungal) that is not under control
• You have not received all recommended vaccinations before starting the study drug (confirm with trial site)
• You are unwilling or unable to follow the procedures required by this study
• You are currently taking part in another clinical trial of an experimental treatment
• You are a woman who is pregnant, planning to become pregnant during the study, or currently breastfeeding
