Who might be able to join this trial:

For the Focus Groups (Year 1) — Veterans:

• You are 18 years old or older
• You are a Veteran who is eligible for VA healthcare
• You speak English
• You have received cancer monitoring, cancer treatment, or lung cancer screening at the San Francisco VA Healthcare System (SFVAHCS) in the past 24 months
• You have smoked cigarettes at some point in the past 24 months
• You have access to Wi-Fi and a device that can make video and audio calls (such as a smartphone, tablet, or computer)

For the Focus Groups (Year 1) — VA Clinical Staff:

• You currently work as a clinical staff member at SFVAHCS
• You have been involved in the care of at least 5 VA cancer or lung cancer screening patients in the past 6 months

For the Pilot Study (Year 2) and Main Trial (Years 3–6) — Veterans:

• You are 18 years old or older
• You are a Veteran currently receiving medical care at SFVAHCS, with at least one clinic visit this calendar year related to cancer
• You speak English
• You are currently enrolled in a VA lung cancer screening program this calendar year, or you have a current cancer diagnosis recorded in your VA medical records
• You are currently smoking at least 1 cigarette per day on average (checked over the past 30 days)
• You are open to trying tools or support to help you quit smoking

Who may not be able to join:

For the Focus Groups (Year 1) — Veterans:

• You have a severe, untreated mental health condition such as psychosis, bipolar disorder, or a serious substance use disorder
• You have had active thoughts of harming yourself or others, or have shown serious behavioral instability, in the past 30 days that could prevent participation
• You have never smoked, or you stopped smoking more than 36 months before signing up
• You do not have access to Wi-Fi or a device that supports video and audio communication

For the Focus Groups (Year 1) — VA Clinical Staff:

• You are unable to commit approximately 1.5 hours for the focus group and questionnaires

For the Pilot Study (Year 2) and Main Trial (Years 3–6) — Veterans:

• You have had an unstable psychiatric or medical condition in the past 30 days — including psychotic disorders, bipolar disorder, memory or thinking disorders, or substance use disorders — as judged by the research team (confirm with trial site)
• You have an untreated, active gambling problem, based on medical records or a screening questionnaire score (confirm with trial site)
• Your cancer has spread to other parts of your body (metastatic cancer), or you are currently receiving end-of-life or palliative care
• You are unable to commit to the time required to take part in the study
• You are currently pregnant or planning to become pregnant during the study (applies to people of childbearing potential between ages 18 and 55)
• You have attempted suicide or had thoughts of suicide with intent to act in the 30 days before enrolling
• You are already taking part in another clinical trial focused on quitting tobacco
