Who might be able to join this trial:

• People who have been fully informed about the study and have signed a written consent form
• Adults aged 18 to 65 years, either men or women who are not pregnant
• People who have been diagnosed through a tissue biopsy with a specific type of nasopharyngeal cancer (non-keratinizing carcinoma of the nasopharynx, WHO type II or III)
• People whose cancer has been staged at a specific advanced level based on tumour and lymph node spread, with no distant spread to other parts of the body (staged as T4N0-1M0 or T1-4N2-3M0 under AJCC 8th edition guidelines — confirm with trial site)
• People who have not yet received any cancer treatment
• People with a general health and activity level rated as fully active or with only minor limitations (ECOG score of 0 or 1)
• People whose blood count results meet minimum levels for white blood cells, haemoglobin, and platelets
• People whose liver function blood test results are within an acceptable range
• People whose kidneys are functioning at an adequate level based on blood test results

Who may not be able to join:

• People whose nasopharyngeal cancer has come back after treatment or has spread to other parts of the body
• People whose biopsy confirms a different type of nasopharyngeal cancer called keratinizing squamous cell carcinoma
• People who have previously had radiation therapy or chemotherapy
• Women who are pregnant, breastfeeding, or of childbearing potential without using effective contraception
• People who test positive for HIV
• People with a known history of another cancer, unless it was a fully cured basal cell skin cancer or early-stage cervical cancer
• People who have previously been treated with certain immune-based therapies, including anti-PD-1, anti-PD-L1, or anti-CTLA-4 medications
• People with an immune system disorder or a history of having received an organ transplant
• People who have taken high-dose steroids, certain cancer antibody treatments, or other immune-suppressing medications within the past 4 weeks
• People with serious problems affecting the heart, liver, lungs, kidneys, or bone marrow
• People with severe or uncontrolled illness or active infections
• People currently taking part in another clinical trial or receiving another experimental treatment
• People who are unable or unwilling to sign the informed consent form
• People for whom the trial treatments would be medically unsuitable for other reasons (confirm with trial site)
• People with a significant personality or mental health condition that affects their ability to make legal or medical decisions
• People who test positive for the hepatitis B surface antigen with a high level of hepatitis B virus detected in their blood
• People who test positive for hepatitis C antibodies, unless a follow-up test shows the hepatitis C virus is not active in their blood
