For patients:
- You are 18 years of age or older
- You speak English and are able to give your own consent to participate

- You are accompanied by someone who is not willing to wait outside during your visit, does not want the visit to be audio recorded, is a minor without a parent or legal guardian present, or is unable to give their own consent (confirm with trial site)
For healthcare providers:
- You work at Duke University and were already involved in an earlier version of this study
- You are planning to leave your current institution within 12 months of joining the study
