For the adolescent:

• The adolescent has a body weight that is either very low (underweight) or in the obese range based on age-related weight charts
• The adolescent has a long-term medical condition (such as type 2 diabetes) that would require more intensive medical support than this trial provides
• The adolescent has health responses on a standard physical activity safety questionnaire that suggest a serious health concern, and a doctor has not approved participation
• The adolescent has a cognitive or developmental condition (such as Down Syndrome) that may make it difficult to understand the program materials, as reported by the parent

For the parent or guardian:

• The parent or guardian has health responses on a standard physical activity safety questionnaire that suggest a serious health concern, and a doctor has not approved participation
• The family has plans to move out of the South Florida area during the follow-up period of the study
