Who might be able to join this trial:

• You must have been diagnosed with osteosarcoma (a type of bone cancer) confirmed by a tissue sample, either at the original diagnosis or when the cancer came back.
• The cancer must have come back (relapsed) at least once after treatment.
• The cancer that has returned must be found only in the lungs (one or both lungs are okay).
• All tumors in the lungs must be removable by surgery without having to remove an entire lung, as decided by a surgeon at the trial site.
• You must have at least one lung tumor of a certain size that can be treated with a targeted form of radiation (called SBRT), plus at least one other tumor that qualifies for surgical removal.
• You must be well enough to carry out most daily activities (measured by a standard health performance score).
• Any side effects from previous treatments must have mostly gone away or be considered stable by your doctor.
• Enough time must have passed since your last cancer treatment — the exact waiting period depends on the type of treatment you received (confirm specific timeframes with the trial site).
• Your bone marrow must be working well enough, shown by blood counts being above certain levels, and you must not have needed a platelet transfusion in the last 7 days.
• Your kidneys must be working well enough (confirmed by a blood or urine test).
• Your liver must be working well enough (confirmed by blood tests).
• Your pancreas must be working well enough (confirmed by a blood test).
• Your thyroid must be functioning normally (confirmed by a blood test).

Who may not be able to join:

• You are currently pregnant or breastfeeding.
• The cancer has spread to areas outside the lungs, such as bones or the brain.
• You have more than a mild build-up of fluid around the lungs.
• You have had radiation treatment to the lungs before.
• You have an active autoimmune disease (where the immune system attacks the body) that has needed systemic treatment in the past 12 months, or a history of a severe autoimmune condition — though some milder conditions such as type 1 diabetes, an underactive thyroid managed with hormones, or certain skin conditions may still be allowed (confirm with trial site).
• You have had a serious heart problem such as a heart attack or stroke within the past 3 months, or you have an unstable heart rhythm or unstable chest pain.
• You have previously received a type of cancer treatment called an immune checkpoint inhibitor, but only if it was permanently stopped because of serious side effects, or if it was given alongside radiation therapy.
• You currently have active tuberculosis.
• You have any medical condition that would make surgery unsafe or prevent you from following the trial's requirements.
• You regularly take medications that suppress your immune system.
• You currently have an infection that is not under control.
• You have previously received a bone marrow or stem cell transplant from another person, or a solid organ transplant (such as a kidney or liver transplant).
• In your doctor's opinion, you may not be able to follow the trial's required procedures.
• You are currently receiving any other experimental or cancer-fighting treatments.
• You have a known history of HIV, hepatitis B, or hepatitis C.
• You currently have or have previously had inflammation of the lungs (pneumonitis).
• You have received a live or weakened vaccine within 30 days before joining the trial.
