Who might be able to join this trial:
The trial has five separate groups, each with different requirements:
- Group 1 – Lean, young adults: People aged 18–35 with a BMI between 18.5 and 24.9 (maintained for at least 12 months)

- Group 5 – Nonagenerians: People aged 90 or older with a BMI of 18.5 or higher (maintained for at least 3 years)
Who may not be able to join:
For Groups 1 and 2 (young adults — lean and overweight):
- People who exercise at moderate to high intensity more than 5 times per week
- People following special diets such as a ketogenic diet or time-restricted eating
- People with significant hormone or metabolism disorders
- People with claustrophobia (fear of enclosed spaces, such as an MRI machine)
- People who use illicit drugs or drink more than 15 alcoholic drinks per week
- People with heart failure, cardiovascular disease, conditions requiring immune-suppressing medications, ongoing inflammatory diseases, or active cancer
- People with metal implants that are not compatible with a 3-Tesla MRI scan
- People who are pregnant or breastfeeding
- Group 2 only: People currently using weight-loss medications such as GLP-1 drugs (e.g. semaglutide), naltrexone, or bupropion
For Groups 3 and 4 (older adults — lean and overweight):
- People following special diets such as a ketogenic diet or time-restricted eating
- People with diabetes or an overactive or underactive thyroid
- People with claustrophobia
- People who use illicit drugs or drink more than 15 alcoholic drinks per week
- People with metal implants that are not compatible with a 3-Tesla MRI scan
- People who had heart failure, cardiovascular disease, immune-suppressing treatment, inflammatory disease, or active cancer before the age of 35
- People with a life-threatening condition or life expectancy of less than one year, or any condition that could affect their safety or ability to participate
- Group 3 only: People who exercise at moderate to high intensity more than 5 times per week
- Group 3 only: People with severe cardiovascular disease, advanced heart failure, or significantly reduced heart pumping function
- Group 4 only: People currently using weight-loss medications such as GLP-1 drugs, naltrexone, or bupropion
For Group 5 (people aged 90 and over):
- People with claustrophobia
- People with significant cognitive impairment (such as severe memory or thinking problems) that would affect their ability to take part
- People with metal implants that are not compatible with a 3-Tesla MRI scan
- People with a life-threatening condition or life expectancy of less than one year, or any condition that could affect their safety or ability to participate
