Who might be able to join this trial:

• People for whom ECT (electroconvulsive therapy) has already been recommended by their primary psychiatrist as a clinically appropriate treatment
• Adults aged 18 to 90 years old, of any gender
• People who currently meet the diagnostic criteria for a major depressive episode with some level of suicidal thoughts, as part of major depressive disorder, bipolar disorder, or schizoaffective disorder
• People who score 2 or higher on the suicidal thoughts item of a standard depression rating scale called the MADRS
• Women who are able to become pregnant must be using an effective form of birth control
• People who are medically stable at the time of the trial
• People whose psychiatric medications are not expected to need changing during the study period (except in urgent situations)
• People who are able to fully understand and take part in the informed consent process

Who may not be able to join:

• People with an unstable or serious medical condition that would significantly increase the risks of ECT or affect memory and thinking
• Women who are pregnant, planning to become pregnant during the study, or currently breastfeeding
• People with a history of a neurological condition, metal in the head, a known brain lesion, or a skull defect that the treating doctor considers a significant risk for ECT or that may affect cognition
• People who have implanted medical devices that would make ECT unsafe
• People currently experiencing delirium or dementia
• People with an active substance use disorder within one week of being enrolled in the study
• People who have had ECT in the past month, or who have previously had a full course of ECT without responding to it, as determined by the treating doctor
