Who might be able to join this trial:

• People aged 18 to 70 years old
• People who have been diagnosed with non-small cell lung cancer (NSCLC) confirmed by laboratory testing of tissue or cell samples, where testing has clearly shown the cancer does not have certain gene changes (EGFR, ALK, or ROS1 mutations) in the main tumour or nearby lymph nodes
• People whose tumour tissue shows very low levels of a protein called PD-L1 (less than 1% of tumour cells)
• People whose NSCLC is at a stage that surgeons consider removable — specifically stages IB (tumour at least 4 cm) through IIIB (with N2 lymph node involvement), based on a specific cancer staging system (AJCC, 7th edition)
• People whose general physical health is rated at 0 or 1 on a standard medical fitness scale, meaning they are fully active or able to carry out light activity
• People who have not previously received any cancer treatment
• People whose cancer can be measured on scans according to standard criteria (RECIST v1.1), and who have tumour tissue available from before treatment to test for PD-L1 levels
• People whose major organs (such as heart, liver, and kidneys) are functioning normally

Who may not be able to join:

• People whose lung cancer is classified as large cell carcinoma, mixed cell lung cancer, or cancer that contains any small cell lung cancer component
• People whose cancer has spread to other parts of the body or cannot be surgically removed
• People whose scans show the tumour has grown into or very close to major blood vessels, where there is considered to be a high risk of serious bleeding, or who have a tumour in the lung that has large hollow or dead tissue areas
• People who have already received any systemic cancer treatment for NSCLC, including chemotherapy, immunotherapy, or experimental treatments
• People who have previously had radiation therapy directed at their NSCLC
• People who have had another type of cancer in the five years before joining this trial (exceptions may apply for certain early-stage or cured cancers, such as cervical carcinoma in situ, cured basal cell skin cancer, or certain early bladder tumours — confirm with trial site)
• People who have a known allergy to the study drug (cardunculizumab) or to any component of the chemotherapy drugs used in this trial
• People who have a severe or uncontrolled medical condition of any kind
• People who have a history of certain lung conditions, including interstitial lung disease, lung inflammation caused by previous drugs or radiation that required steroid treatment, or any currently active interstitial lung disease
• People who have had a stroke, mini-stroke (TIA), or blood clot in the lungs within the past 6 months
• People who currently have nerve damage in their hands or feet rated at grade 2 or higher on a standard medical scale (unless caused by a physical injury)
• People who require complete removal of the entire right lung as part of their surgery
• People who have had major surgery or a serious physical injury within the 14 days before joining, where the effects have not yet fully resolved
• People who are currently taking part in another clinical trial, or whose last clinical trial treatment ended less than 4 weeks ago
• People with a known history of severe allergic reactions to other monoclonal antibody medicines
• People who are pregnant or currently breastfeeding
• People with a confirmed history of serious neurological or psychiatric conditions, including epilepsy or dementia
• People who, in the opinion of the trial investigators, have other medical, laboratory, family, or social factors that could affect their safety or the reliability of trial data (confirm with trial site)
