Who might be able to join this trial:

• You have been diagnosed with OCD (Obsessive-Compulsive Disorder) based on standard medical guidelines (DSM-5).
• You are between 18 and 65 years old.
• You have had OCD for more than 3 years.
• You have tried at least three different serotonin-based medications (including one called clomipramine) at high doses for more than 12 weeks without improvement, or you experienced serious side effects from these medications.
• You have tried a talking therapy called CBT (Cognitive Behavioural Therapy) without success, or you have repeatedly declined to try it.
• Your OCD symptoms are rated as severe based on a standard assessment tool called the Y-BOCS (score of 25 or higher).
• Your OCD significantly affects your daily life, such as making it impossible to work, study, or take care of yourself.
• You are able to understand what the trial involves and are able to sign a consent form, and your self-awareness is not severely affected by your condition.

Who may not be able to join:

• You have been diagnosed with bipolar disorder, schizophrenia, schizoaffective disorder, or another psychotic condition.
• You have a severe personality disorder.
• You have a serious physical illness or a brain disease caused by an underlying physical condition.
• You have had a diagnosis of alcohol or drug abuse or dependence within the past year.
• You have health conditions that make surgery unsafe for you, or you are unable to stop taking blood-thinning medications.
• A brain MRI scan taken before the study shows significant abnormalities in the structure of your brain.
• You have any reason that means you cannot safely have an MRI scan (not counting the device that would be implanted as part of this trial).
• You are currently pregnant or breastfeeding.
• You are already taking part in a different research study that is unrelated to this one.
• You have any current or potential circumstances — including lack of family support, medical, psychological, social, or geographical factors — that could put you at risk or prevent you from completing the study (confirm with trial site).
