Who might be able to join this trial:

• You are an adult who has been diagnosed with Psoriatic Arthritis (PsA) using a recognised set of medical criteria called CASPAR
• You have one or more swollen joints and/or one or more painful, tender spots where tendons or ligaments attach to bone (enthesitis)
• You have one or more raised, scaly skin patches caused by psoriasis
• Your regular medications have not changed in the last three months, and you have not used steroid tablets or long-term steroid treatments in the last 8 weeks
• You are overweight or obese, meaning your Body Mass Index (BMI) is 25.0 or above (or 23.0 or above if you are of Asian background)
• Your doctor (rheumatologist) has already decided to start you on a medication called Guselkumab for your PsA
• A special scan of your liver shows a higher-than-normal amount of fat in your liver, consistent with a condition called NAFLD (non-alcoholic fatty liver disease), after other liver conditions have been ruled out

Who may not be able to join:

• You have previously taken certain types of medications for inflammatory conditions, specifically IL-12/23 inhibitors, IL-17 inhibitors, JAK inhibitors, or TYK2 inhibitors, or you have used more than two TNF inhibitor treatments in the past (confirm with trial site)
• You have Hepatitis B, Hepatitis C, or any other known chronic liver disease from another cause
• You have certain inherited or immune-related liver conditions such as autoimmune hepatitis, primary biliary cirrhosis, Wilson disease, alpha-1-antitrypsin deficiency, or haemochromatosis (confirm with trial site)
• You have liver scarring (cirrhosis) or have had complications from serious liver disease such as fluid build-up in the abdomen, confusion caused by liver failure, or enlarged veins in the oesophagus
• You have had weight-loss bypass surgery or have taken certain medications known to cause liver fat build-up (such as high-dose steroids, tamoxifen, or amiodarone) in the past 6 months
• You drink alcohol regularly or heavily — more than 30g per day if male, or more than 15g per day if female — and have done so for more than 2 years at any point in the last 10 years
• Your kidneys are not working well enough, based on a specific blood test result (creatinine above 2.0 mg/dL)
• You are pregnant, breastfeeding, or planning to become pregnant
• Your life expectancy is estimated to be less than 5 years
• You have a known HIV infection
