Who might be able to join this trial:
- People who are 18 years of age or older
- People who speak English or Spanish
- People who have been diagnosed with diabetes and have had a blood sugar control test (called an A1C) result of 8.0% or higher in the past two years, OR people who have been diagnosed with high blood pressure and have had a recorded top blood pressure reading above 140 mmHg
- People who have had at least 2 visits at a participating San Francisco Health Network (SFHN) primary care clinic in the last 24 months
Who may not be able to join:
- People who already have a higher-than-average ability to use digital health tools, as measured by a specific digital health literacy score (confirm with trial site)
- People who have serious health conditions that would make it unsuitable to focus on managing long-term illness through telehealth, such as a terminal illness with a short life expectancy, or a severe mental illness
- People who do not have any working phone number
- People whose vision or hearing difficulties would prevent them from using telehealth services for managing a long-term condition
- People who have difficulty with memory or thinking that makes it hard to understand and restate the goals of the study during the consent process
- People who are currently pregnant
