Who might be able to join this trial:

• People who have had surgery to remove a kidney and ureter (called radical nephroureterectomy) and whose lab results confirmed cancer of the renal pelvis or ureter, at an advanced stage (classified as pT3-4 or pN+ under standard staging guidelines)
• People whose kidneys are not filtering blood well enough (low eGFR below 60), or who have another health condition that means chemotherapy is not suitable for them
• People aged between 18 and 80 years old
• People who have had a CT scan of the abdomen and pelvis within 4 weeks before joining the trial
• People who have not had any other cancer in the past 5 years, except for certain skin cancers (non-melanoma) or ductal carcinoma in situ of the breast
• People who are willing to attend all required check-ups and follow-up visits for the study, and who are willing to sign a written consent form
• People with an expected survival of more than 6 months and a general health score above 70 (on the Karnofsky scale, which measures ability to carry out daily activities) (confirm with trial site)
• People whose blood test results meet minimum levels for white blood cells, a type of white blood cell called neutrophils, platelets, and haemoglobin (confirm with trial site for exact values)

Who may not be able to join:

• People whose cancer had already spread to distant parts of the body at the time of surgery, or whose surgery did not remove all visible cancer
• People who have previously had radiotherapy to the pelvis or abdomen, a history of inflammatory bowel disease, or prior chemotherapy treatment
• People who are pregnant or breastfeeding, or women of childbearing age who are not using a reliable form of contraception
• People who have had another cancer diagnosis in the past (except non-melanoma skin cancer, cervical cancer in situ, or cancers that were fully treated more than 5 years ago)
• People who have lost more than 10% of their body weight in the past 6 months
• People who have an active infection, or a pre-existing bleeding disorder (confirm with trial site)
• People with significant heart or cardiovascular conditions, such as poorly controlled high blood pressure, unstable chest pain (angina), certain levels of heart failure, irregular heart rhythms causing symptoms, or significant circulation problems in the limbs
• People whose psychological, family, or social circumstances mean they are not in a position to give informed consent
