Who might be able to join this trial:

• People who have been diagnosed with a specific type of cancer called squamous cell carcinoma, confirmed by a laboratory test, located in the mouth, throat, voice box, lower throat, sinuses, or neck lymph nodes with an unknown starting point.
• People whose treatment plan includes radiation therapy after surgery, with the goal of curing the cancer.
• People with certain higher-risk features found during surgery (such as cancer spread to multiple lymph nodes, cancer growing into nerves or blood vessels, cancer cells found at or near the edges of the removed tissue, or cancer that has broken out of the lymph nodes), with the specific risk factors required depending on the type and location of the cancer (confirm with trial site).
• People who have had their visible tumour completely removed by surgery, where the gap between that surgery and the planned start of radiation is more than 42 days but no more than 112 days.
• People aged 18 or older at the time of joining the trial.
• People who are in generally good physical condition, able to carry out normal daily activities with little or no limitation (what doctors call ECOG performance status 0–1).
• People who are able to understand and sign a consent form (available in English or Spanish) and complete study-related tasks.

Who may not be able to join:

• People whose head and neck cancer has come back after previously receiving radiation therapy, or after surgery followed by radiation therapy.
• People who have developed a second, separate head and neck cancer after already being treated for a previous one.
• People who have had radiation treatment to the head or neck area before, where any part of the new planned radiation field would overlap with an area that was previously treated.
• People who currently have another active cancer (other than certain common skin cancers or very early-stage cancers that have not spread).
• People whose head and neck cancer has spread to distant parts of the body or to lymph nodes outside the head and neck area.
• People where the time between their primary surgery and the planned start of radiation therapy is more than 112 days.
• People with serious uncontrolled health conditions, such as an active infection, uncontrolled heart failure, unstable chest pain, irregular heartbeat, or mental health or personal circumstances that would make it difficult to follow the study requirements.
• People who are not considered suitable candidates for radiation therapy by their treating doctor — for example, people who are pregnant or planning to become pregnant, or anyone with another medical reason that makes radiation therapy unsuitable.
