Who might be able to join this trial:
Group 1 (Newly diagnosed patients):
- You have been newly diagnosed with a specific type of childhood brain tumor (pediatric high-grade glioma, diffuse midline glioma, or diffuse intrinsic pontine glioma), confirmed by biopsy or MRI scan
- You weigh more than 10 kg (about 22 lbs)
- You are well enough to carry out at least some daily activities (as measured by a standard medical scale)
- If you have seizures, they have been stable — meaning no seizures for at least 7 days and no need for new or stronger seizure medicines
- Your liver, kidneys, and blood counts (including red blood cells, white blood cells, and platelets) are within acceptable ranges
Group 2 (Relapsed, progressive, or post-radiation patients):
- Your brain tumor (high-grade glioma, diffuse midline glioma, diffuse intrinsic pontine glioma, or medulloblastoma) has returned or progressed, OR you have finished standard radiation and have not yet had progression
- You are between 2 and 25 years old
- You weigh more than 10 kg (about 22 lbs)
- You have previously received standard treatments such as surgery, radiation, and/or chemotherapy
- You have recovered from the side effects of your previous treatments
- You have not previously taken the study drug atovaquone
- Your seizures, if any, have been stable for at least 7 days with no need for new or stronger seizure medicines
- Your liver, kidneys, and blood counts are within acceptable ranges
- Patients with cancer that has spread to other parts of the body may be eligible for Group 2
Who may not be able to join:
- You have a serious ongoing illness unrelated to your cancer (confirm with trial site)
- You have had an allergic reaction to atovaquone or a very similar medication
- Your tumor has spread to other parts of the body (Group 1 only — this is allowed in Group 2)
- You have uncontrolled seizures, or your seizure medicines recently needed to be increased or changed
- You have not yet fully recovered from the effects of a recent surgery
- You are pregnant or breastfeeding; girls who have started their period must take a pregnancy test before joining
- You are able to become pregnant or father a child and are not willing to use effective birth control during treatment and for 3 months after treatment ends
- (Group 2 only) You have not waited long enough after your last cancer treatment — specific waiting periods apply depending on the type of treatment you received (confirm exact timeframes with trial site)
- (Group 2 only) You have had a stem cell transplant less than 12 weeks ago
- (Group 1 only) You have received any anti-cancer treatment other than radiation therapy
- You have had significant bleeding inside your tumor
