Who might be able to join this trial:
- People who have been diagnosed with head and neck squamous cell carcinoma (a type of head and neck cancer confirmed by laboratory testing) that has already received some initial treatment and can be surgically removed.
- People whose cancer is classified as stage III to IVB (using the AJCC 8th edition staging system), except for cases of HPV-positive cancer of the oropharynx (the middle part of the throat).
- People whose medical team — including ear, nose and throat specialists and radiation specialists — has assessed the tumour as removable or close to removable, and suitable for a specific type of targeted radiation treatment before surgery (called SBRT).
- People with a general health score of 70 or above on the Karnofsky Performance Scale, which measures a person's ability to carry out daily activities.
- People aged between 18 and 70 years old.
- People whose major organ functions (such as liver and kidneys) meet the levels required by the trial's testing standards.
- People who are willing to take part voluntarily and have signed a consent form.
Who may not be able to join:
- People who have previously had surgery on the head or neck area, except for diagnostic biopsies of the primary tumour or nearby lymph nodes.
- People who have previously received chemotherapy for any reason, radiation therapy to the head or neck, targeted drug therapies, certain immunotherapy drugs (including anti-PD-1, anti-PD-L1, anti-PD-L2, anti-CTLA-4, or similar treatments), or cell-based therapies.
- People who are pregnant or breastfeeding.
- People who have had, or currently have, another type of cancer.
- People who have a serious illness that is not well controlled.
- People with significant heart, brain, lung, or other major organ problems — including uncontrolled high blood pressure, heart attack history, certain heart rhythm problems, heart failure, abnormal blood clotting, a known tendency to bleed, or significant protein in the urine (confirm specific thresholds with trial site).
- People who have been on corticosteroid medications (such as prednisone at more than 10mg per day) for 30 days before starting the trial, or who have an active autoimmune disease that has needed systemic treatment in the past 2 years.
- People who have had a history of lung inflammation (not caused by infection) that required corticosteroid treatment within the past year, or who currently have a chronic lung condition called interstitial lung disease.
- People with an active infection, such as tuberculosis, that requires ongoing treatment.
- People with a known history of HIV infection.
- People with untreated active hepatitis B, or active hepatitis C infection (note: people with hepatitis B may still be considered eligible under certain conditions — confirm with trial site).
- People with a history of substance abuse or mental health conditions that cannot be appropriately managed.
- People currently taking part in another treatment-based clinical trial.
- People where the trial investigators identify other circumstances that could interfere with the study or its results.
