Who might be able to join this trial:

• People aged 14 to 55 years old, of any gender.
• People who have received a stem cell transplant from a donor (any donor type) as treatment for a type of blood cancer called T-LBL or T-ALL.
• People whose physical functioning is rated at a certain level of ability (ECOG score of 0–2, meaning able to carry out at least some daily activities — confirm with trial site).

• People who had at least one of the following high-risk features at any point in their illness: age 35 or older at first diagnosis; very high white blood cell count at first diagnosis; elevated LDH blood levels at first diagnosis; cancer cells found in the bone marrow at first diagnosis; a large mass in the chest area (10 cm or longer) at first diagnosis; a specific type of immune cell profile called ETP; cancer that did not respond well enough during early chemotherapy; signs of remaining disease before transplant detected by bone marrow testing, molecular testing, or PET-CT scan; certain specific gene changes linked to higher risk, as defined by specialist guidelines (confirm with trial site).

Who may not be able to join:

• People whose cancer spread to the central nervous system (brain or spinal cord) at any point during their illness.
• People whose cancer had not gone into full remission before their transplant.
• People for whom a specific targeted drug treatment is available and suitable.
• People whose cancer did not respond to, or got worse during, a type of treatment called a BCL-2 inhibitor before transplant.
• People known to have an allergy to demethylating drugs or venetoclax.
• People experiencing a moderate or severe acute immune reaction against the transplant (Grade 2 or higher acute GVHD).
• People experiencing moderate to severe long-term immune reaction against the transplant (moderate to severe chronic GVHD).
• People whose cancer has come back after the transplant, or whose body has rejected the transplant, or where fewer than 95% of bone marrow cells are from the donor.
• People whose blood counts fall below the minimum levels required (white blood cell or platelet counts too low).
• People with serious problems affecting major organs, including significantly elevated liver enzymes or bilirubin levels, or reduced kidney function.
• People with a serious active infection.
• People who are pregnant or breastfeeding.
• People currently taking part in another clinical drug trial.
• People who, in the assessment of the research team, have other conditions that could put them at risk.
