Who might be able to join this trial:

• People who identify as African American or Black
• People who have had two kidney function test results (called eGFR) below 29, taken at least 90 days apart but within the past 2 years — OR — people whose kidney failure risk score shows a 10% or greater chance of kidney failure within the next 2 years
• People who have had at least one visit at Emory Healthcare — either an outpatient appointment, an emergency room visit, or a hospital stay — within the past 2 months
• People who are willing to follow all study requirements and are available for the full duration of the study

Who may not be able to join:

• People who are currently on dialysis
• People who are currently receiving hospice care or other end-of-life care for a terminal illness
• People who are currently on a transplant waiting list, have been referred for a transplant evaluation, or have completed a transplant evaluation visit within the past 2 years
• People who have previously received a kidney transplant or any other solid organ transplant
• People who are currently undergoing treatment for cancer
• People whose primary language is not English
• People who are already taking part in another treatment or intervention study at the time of enrollment
• People who are currently pregnant or planning to become pregnant at the time of recruitment
