Who might be able to join this trial:
- You are 18 years of age or older
- You speak English fluently
- You are able to give your agreement to participate in writing and verbally
- You have been diagnosed with OCD (Obsessive-Compulsive Disorder) through a formal clinical interview
- You are currently experiencing significant OCD symptoms
- You have had OCD symptoms for at least 1 year
- Your OCD is your main or one of your main mental health concerns (not secondary to another condition)
- You have been on a stable mental health treatment plan for at least 8 weeks, or you are not currently receiving any treatment for OCD
- Your OCD symptoms score at least 18 on a specific OCD symptom rating scale called the DOCS (confirm with trial site)
Who may not be able to join:
- You currently have a serious problem with substance use (such as drugs or alcohol)
- You are currently having thoughts of suicide
- You have a history of bipolar disorder or a psychotic disorder (such as schizophrenia)
- You have significant developmental disabilities
- You have ever lost consciousness for more than 10 minutes
- You have a history of traumatic brain injury
- You have a major neurological disease (such as epilepsy or Parkinson's disease) (confirm with trial site)
- You have a positive pregnancy test
- You have a medical reason why you cannot have brain stimulation or an MRI scan (confirm with trial site)
- You have started a new psychological therapy (such as talking therapy) within the past 8 weeks
- You take a daily anti-anxiety medication such as a benzodiazepine (for example, Valium or Xanax)
- (For healthy volunteers without OCD only) You currently meet the criteria for any mental health disorder, or you are taking any psychiatric medications

Primary endpoints
Blood oxygen level-dependent (BOLD) response to create functional brain activation maps; Skin conductance response (SCR) will be measured by collecting electrodermal activity data from the left hand; Threat expectancies will be measured by asking participants to rate the probability of receiving a shock using a 0-100 scale with zero equal to 0% chance and 100 equal to 100% chance; higher values are indicative of worse anxious reactivity; Functional brain connectivity will be measured blood oxygen level dependent (BOLD) response
