Who might be able to join this trial:

• A tissue sample (biopsy) has confirmed a diagnosis of the most aggressive type of brain tumour, called glioblastoma (WHO grade 4, IDH wild-type), and this has been verified by a central review team.
• The diagnosis is newly confirmed, and no more than 8 weeks have passed since the surgery to remove or sample the tumour.
• The person was aged 65 or older at the time of their glioblastoma diagnosis.
• The person is reasonably able to carry out daily activities, as measured by a standard scale used by doctors (ECOG score of 0, 1, or 2).
• Blood test results taken within the 45 days before joining the trial show that the blood, kidneys, and liver are functioning at adequate levels (specific thresholds apply — confirm with trial site).
• If the person has been taking steroid medications, the dose must have been stable or reducing for at least 14 days before joining the trial.

Who may not be able to join:

• People whose glioblastoma has come back after previous treatment (recurrent disease).
• People whose cancer has spread to the spine, the lining of the brain or spinal cord, or other distant parts of the body.
• People who have another active brain or central nervous system cancer that requires treatment.
• People who have a second cancer that is either not fully treated, still requires ongoing treatment, or is expected to result in a life expectancy of less than one year based on general population data for that cancer type and stage.
• People with serious other medical illnesses or psychiatric conditions that, in their treating doctor's opinion, would prevent them from completing the trial treatment.
• People who do not have the mental capacity to give informed consent to participate.
• People who are unable to receive gadolinium (a contrast dye used in MRI scans).
• People who have previously had radiation therapy to the head, neck, or skull area, or to any other area where the radiation fields would be expected to significantly overlap with the planned treatment area.
• People who have previously received chemotherapy, radiation-sensitising drugs, or any experimental cancer drug for a prior head, neck, or central nervous system cancer, or any investigational drug for glioblastoma.
• People who have received, or plan to receive, any other non-surgical local treatment in the same area where radiation is planned.
• People who have a serious active infection (such as a wound infection needing intravenous antibiotics) or another acute medical condition at the time of joining the trial that would interfere with receiving the treatment.
