Who might be able to join this trial:

• People who have signed an informed consent form agreeing to take part
• People aged 18 years or older
• People who have been diagnosed with type 2 diabetes for at least 12 months
• People who have been using insulin therapy for at least 6 months
• People with chronic kidney disease (CKD) that has been present for at least 6 months — either with a certain level of reduced kidney filtering ability (eGFR 30–59), or with a higher-than-normal amount of protein in the urine along with at least some kidney filtering ability remaining (confirm with trial site for exact values)
• People whose blood sugar control measure (HbA1c) is below 12%, based on a blood test taken at the screening visit
• People who are currently taking certain diabetes medications known as SGLT2 inhibitors and/or GLP-1 receptor agonists, have previously been offered these medications, or have a medical reason why these medications are not suitable for them
• People who are willing to wear study devices and follow the study's instructions
• People who are able to understand and agree to the study requirements

Who may not be able to join:

• People who have type 1 diabetes
• People who currently use an insulin pump
• People who currently use any automated closed-loop insulin delivery system
• People whose kidney disease is caused by a condition other than typical CKD, such as polycystic kidney disease, kidney inflammation, or certain birth-related urinary tract conditions (as recorded in medical notes)
• People with a known or suspected allergy to insulin
• People who are pregnant, planning to become pregnant, or currently breastfeeding
• People with severe vision impairment
• People with severe hearing impairment
• People who have had two or more serious low blood sugar episodes in the past 6 months
• People with a documented allergy to the adhesive in medical plasters or patches
• People with serious skin conditions on areas of the body where a glucose sensor would need to be placed
• People with a physical or psychological condition, or taking medications, that the study doctor believes could affect how the study runs or how results are interpreted
• People who have had a heart attack, a heart procedure (such as a stent), a stroke, or a hospital admission for a certain type of heart failure within the past 3 months
• People who have had a kidney transplant and are still taking anti-rejection medications
• People with known or suspected issues with following medical instructions, or with drug or alcohol misuse
• People who are unable to follow study procedures, for example due to language barriers
• People who are not able to give informed consent
• People who cannot undergo MRI scanning — for example, due to having a pacemaker or other metal implant in the body, or due to severe claustrophobia
• People who do not wish to be told about any unexpected health findings discovered during imaging or other study-related tests
• People who have taken part in another clinical trial involving an experimental drug or medical device in the 30 days before or during this study
• People who have previously been enrolled in this same study
• The principal investigator running the trial, along with their family members, employees, or other people who depend on them
