Who might be able to join this trial:

This trial has four separate groups (called cohorts). Each group has its own criteria.

Cohort I – Upper urinary tract cancer (kidney pelvis or ureter) after major surgery:

Who might be able to join this trial:

• People aged 18 or older who are able to legally consent for themselves at the time of signing the consent form
• People who can provide a urine sample for a specific cancer monitoring test (called MRD testing) at a follow-up visit after surgery
• People whose cancer has been confirmed by pathology as a urothelial (bladder lining-type) cancer starting in the upper urinary tract (kidney pelvis or ureter), which may contain some other cell types but no more than 50%, and whose cancer has reached a certain advanced stage (pT3, pT4, or has spread to nearby lymph nodes)

Who may not be able to join:

• People whose pathology results after major surgery do not meet the required criteria
• People who are not expected to be able to attend regular check-up visits at the trial centre within the first year after surgery
• People who are unwilling to undergo regular bladder camera examinations (cystoscopy) and/or imaging scans (at least one of CT, MRI, or PET-CT, with timing and type decided by the treating doctor)
• People who already have confirmed cancer recurrence or spread (metastasis)
• People who currently have another active cancer, or who have had another cancer within the past 5 years (not including certain skin cancers or early cervical cancer that has been successfully treated)

Cohort II – Bladder cancer (non-muscle-invasive or early stage) after surgical removal:

Who might be able to join this trial:

• People aged 18 or older who are able to legally consent for themselves at the time of signing the consent form
• People who can provide a urine sample for centralised testing at a follow-up visit after surgery
• People whose cancer has been confirmed by pathology as a urothelial (bladder lining-type) cancer starting in the bladder, which may contain some other cell types but no more than 50%, and whose cancer stage is T1, Ta, or Tis (carcinoma in situ — a very early, non-invasive form)

Who may not be able to join:

• People whose pathology results after bladder surgery (TUR) do not meet the required criteria
• People who are not expected to be able to attend regular check-up visits at the trial centre within the first year after surgery
• People who are unwilling to undergo regular bladder camera examinations (cystoscopy) and/or imaging scans (type and timing decided by the treating doctor)
• People who already have confirmed cancer recurrence or spread
• People who currently have another active cancer, or who have had another cancer within the past 5 years (not including certain skin cancers or early cervical cancer that has been successfully treated)

Cohort III – Muscle-invasive bladder cancer treated with chemotherapy before bladder removal surgery:

Who might be able to join this trial:

• People aged 18 or older who are able to legally consent for themselves at the time of signing the consent form
• People who can provide urine samples for centralised testing before each round of pre-surgery treatment (neoadjuvant therapy)
• People whose cancer has been confirmed by pathology as a urothelial (bladder lining-type) cancer starting in the bladder, which may contain some other cell types but no more than 50%, and whose cancer is classified as muscle-invasive (stage T2 or higher, including cases where imaging suggests muscle invasion even if pathology showed T1)
• People who undergo full bladder removal surgery (radical cystectomy) after pre-surgery treatment — including removal of the bladder, prostate, and seminal vesicles in men, or bladder and uterus in women
• People who have lymph node removal as part of surgery, covering at least the standard area defined for bladder cancer (confirm with trial site for specific details)
• People who have at least one measurable area of cancer visible on imaging scans before pre-surgery treatment begins, assessed using standard criteria called RECIST 1.1

Who may not be able to join:

• People whose pathology results from bladder surgery (TUR) do not meet the required criteria
• People who do not undergo full bladder removal surgery or whose surgery does not cover the required area
• People who receive fewer than 2 rounds of pre-surgery treatment
• People who do not undergo regular imaging scans during pre-surgery treatment
• People who do not have any measurable cancer visible on scans before pre-surgery treatment
• People who already have confirmed cancer spread to distant parts of the body
• People who currently have another active cancer, or who have had another cancer within the past 5 years (not including certain skin cancers or early cervical cancer that has been successfully treated)

Cohort IV – Muscle-invasive bladder cancer treated with bladder-preserving therapy (TMT):

Who might be able to join this trial:

• People aged 18 or older who are able to legally consent for themselves at the time of signing the consent form
• People who can provide urine samples for centralised testing at each follow-up visit
• People whose cancer has been confirmed by pathology as a urothelial (bladder lining-type) cancer starting in the bladder, which may contain some other cell types but no more than 50%
• People who have imaging done at the trial centre to assess the cancer before starting bladder-preserving treatment (called TMT — a combination of treatments aimed at keeping the bladder)
• People who, after completing TMT treatment, are assessed by their doctor as having had a complete response (no remaining detectable cancer), allowing the bladder to be kept without needing further surgery to remove it

Who may not be able to join:

• People whose pathology results from bladder surgery (TUR) do not meet the required criteria
• People who are not expected to be able to attend regular check-up visits at the trial centre within the first year after treatment
• People who do not have imaging done at the trial centre before treatment begins
• People who are unwilling to undergo regular imaging scans during follow-up (type and timing decided by the treating doctor)
• People who still have visible cancer remaining, or who already have confirmed cancer spread
• People who currently have another active cancer, or who have had another cancer within the past 5 years (not including certain skin cancers or early cervical cancer that has been successfully treated)
