Who might be able to join this trial:
- You are between 18 and 80 years old.
- You have been diagnosed with chronic kidney disease, meaning your kidneys are not filtering blood at full capacity.
- You have chronic heart failure with a weakened heart pump (your heart is pumping at 50% or less of its normal capacity), and your symptoms are at a moderate level or worse.
- Your blood potassium level has been at or below 4.8 mmol/L in the past month.
- If you are a woman who could become pregnant, you had a negative pregnancy test at your first study visit and agreed to use reliable birth control during the trial.

Who may not be able to join:
- You are currently experiencing sudden or acute kidney failure.
- Your blood pressure is too low (below 90 mmHg systolic at screening, or below 95 mmHg between dialysis sessions).
- Your breathing problems are caused mainly by a lung condition rather than heart failure, or you have right-sided heart failure caused by a lung disease.
- You have had a heart attack or stroke within the past 3 months.
- You have a history of a serious allergic reaction causing sudden swelling (angioedema).
- You are on haemodialysis fewer than 3 times per week.
- Your blood test results at screening show significant abnormalities, such as potassium levels that are too high or too low, very low sodium, or signs of liver strain (confirm with trial site).
- You have had a kidney transplant in the past, or you are planning to have one within the next 12 months.
- You are taking certain medications used to treat a lung blood pressure condition, such as sildenafil, during the study period.
- You have a known allergy to the study treatment or any of its ingredients.
- You have another serious illness, such as active cancer, or a condition that means you are not expected to live for at least 12 months.
- You are currently pregnant or breastfeeding, or you plan to become pregnant during the trial.
