Who might be able to join this trial:
- Adults aged 18 or older who speak English or Spanish
- People with a body mass index (BMI) of 30 or higher (a measure of weight relative to height that falls in the "obese" category)

Who may not be able to join:
- People who are currently pregnant
- People who have a cognitive impairment that would affect participation
- People who speak a language other than English or Spanish
- People who have not had a primary care clinic visit in the past 2 years
- For the weight-related part of the trial: people who have previously had weight-loss surgery, or who are planning to have it within the next 6 months, or who are currently enrolled in an intensive lifestyle program
- For the bowel cancer screening part: people who have been diagnosed with bowel (colorectal) cancer, ulcerative colitis, or Crohn's disease
- For the cervical cancer screening part: people who have been diagnosed with cervical cancer, or who have had a hysterectomy (surgical removal of the uterus)
- For the mental health part: people who are currently experiencing thoughts of suicide, have an active substance use disorder, have a history of schizophrenia or bipolar disorder, or are already receiving mental health therapy (having had sessions in the past 3 months or an upcoming appointment within the next month)
