Who might be able to join this trial:

• You are a woman aged 18 to 75 years with breast cancer that has been confirmed by a tissue sample (biopsy).

Who may not be able to join:

• You have a significant build-up of fluid around your lungs, abdomen, or heart that is not controlled and needs to be drained repeatedly.
• Scans show that the tumour is growing into major blood vessels in a way that could cause serious bleeding.
• You have symptoms such as a stiff neck, seizures, or memory/thinking problems, or your cancer has spread to the lining of the brain (leptomeningeal metastasis), confirmed by scans or spinal fluid testing.
• You have high calcium levels in your blood that are uncontrolled or causing symptoms, or you need ongoing medication to manage this.
• You have previously had radiotherapy to your brain.
• You have previously been treated with a drug called trastuzumab deruxtecan (also known as T-DXd or Enhertu).
• You have a history of another cancer for which you received treatment (with some exceptions for certain skin cancers or early-stage cervical changes — confirm with trial site).
• You had major surgery unrelated to your breast cancer within the 4 weeks before joining, or have not fully recovered from such surgery.
• You have a known autoimmune disease (with some exceptions for well-managed thyroid conditions or stable Type 1 diabetes — confirm with trial site).
• You have a lung condition such as interstitial lung disease, non-infectious inflammation of the lungs, or other uncontrolled conditions such as pulmonary fibrosis or active pneumonia.
• You have HIV, AIDS, active hepatitis B or C, both hepatitis B and C together, autoimmune hepatitis, or active tuberculosis within the past year.
• You had a serious infection requiring hospital admission within the 4 weeks before starting the trial, or have an active infection needing antibiotic treatment, or have had an unexplained high fever during the screening period.
• You have had or are planning to have a bone marrow transplant or an organ transplant from another person.
• You have a serious heart condition, such as heart failure, dangerous irregular heartbeats, chest pain requiring medication, significant heart valve disease, a previous major heart attack shown on an ECG, or poorly controlled high blood pressure.
• You have a known allergy to the trial drug or its ingredients, or have had a severe allergic reaction to similar medicines in the past.
• You are pregnant, breastfeeding, have a positive pregnancy test at the start of the trial, or are unwilling to use effective contraception during the study.
• You have a confirmed neurological or psychiatric condition such as epilepsy or dementia, or a known history of alcohol or substance misuse.
• Your doctor feels there is any other reason that makes it unsuitable for you to take part in this trial.
