Who might be able to join this trial:

• You previously had surgery to completely remove early-to-middle stage non-small cell lung cancer (NSCLC), followed by treatment with a drug called osimertinib after surgery.
• Your cancer has now come back at a distant site in the body (meaning it has spread beyond where it originally was), and this is a new diagnosis of recurrence.
• You either finished the full 3 years of osimertinib after surgery, or stopped it early for a reason other than your cancer coming back.
• At least 6 months have passed between when you stopped osimertinib and when your cancer came back.
• Your lung cancer is of a specific type (non-squamous) confirmed by a tissue or cell sample — if a new biopsy is not possible, your doctor's confirmation of recurrence may be accepted.
• You have not received any systemic (whole-body) cancer treatment since your post-surgery therapy, though local treatments like radiation or surgery are allowed.
• Your brain metastases (if any) are not causing symptoms or are considered stable.
• You are at least 18 years old.
• Your lung cancer has a specific gene mutation (EGFR Exon 19 deletion or Exon 21 L858R), confirmed by a blood test and/or tissue test before joining the study.
• Your general health outlook is expected to be more than 12 weeks at the time you start the trial.

Who may not be able to join:

• Your cancer has only come back in the same local or nearby area, rather than spreading to a distant part of the body.
• You have pressure on your spinal cord, or brain metastases that are causing symptoms.
• You have a history of a lung condition called interstitial lung disease (ILD), or lung inflammation caused by radiation or drugs that needed steroid treatment.
• You stopped osimertinib after your surgery because it caused serious side effects.
• You previously took a different type of targeted therapy drug (called an EGFR-TKI, other than osimertinib) after your surgery.
• You have severe or ongoing nausea, vomiting, digestive diseases, difficulty swallowing, or have had major bowel surgery that could prevent you from properly absorbing the study medication.
• You have had an allergic reaction to osimertinib or similar medicines in the past.
• You have medical reasons why you cannot receive the chemotherapy drugs pemetrexed or cisplatin/carboplatin (confirm with trial site).
