Who might be able to join this trial:
- People aged 18 or older, of any gender, who have signed and dated a consent form before entering the trial
- People with heart failure (regardless of how well the heart is pumping) and a certain level of kidney function, as measured by a blood test called eGFR, falling between 25 and 90
- People with established heart or blood vessel disease and an eGFR between 25 and 60
- People with established heart or blood vessel disease AND type 2 diabetes and an eGFR between 25 and 90
- People whose blood potassium level is at or below 5.0 mmol
- People who are currently on a medication called Empagliflozin, or whose treating doctor considers them eligible for treatment with it
- People who have not used a specific type of heart/blood pressure medication called an MRA or aldosterone synthase inhibitor in the 6 months before joining
- People who have been on stable doses of other recommended heart or kidney medications for at least 4 weeks before joining
- People who are being managed as outpatients (not currently admitted to hospital)
Who may not be able to join:
- People who are unable to understand or sign a consent form
- People who have a strong medical reason why they cannot take a type of medication called an aldosterone antagonist
- People who have a strong medical reason why they cannot take a type of medication called an SGLT2 inhibitor
- People who have been hospitalised for heart failure, or had a heart attack, heart surgery, stroke, or mini-stroke in the 90 days before joining
- Women who are pregnant, breastfeeding, or planning to become pregnant during the study period
