Who might be able to join this trial:
For patients:
- People who are Singapore citizens or permanent residents, aged 21 or older
- People who have been diagnosed with the most advanced stage of chronic kidney disease (stage 5) and are currently on dialysis
- People who have been identified by National Kidney Foundation healthcare professionals as suitable for a serious illness conversation, based on one or more of the following: their health has been getting worse with a life-limiting condition; they are struggling to tolerate dialysis; they have had two or more unplanned hospital admissions due to kidney-related problems in the past 6 months; they are already known to palliative or conservative care services; or they have been assessed as having a low level of physical ability using a specific frailty scoring tool (confirm with trial site)
- People whose memory and thinking abilities are within the normal range, based on a standard cognitive test score of 24 or above (as recorded in medical notes)
- People who are able to speak and read English, Mandarin, or Malay
- People who are willing to have their conversations audio-recorded
For caregivers:
- People aged 21 or older
- Family members who are the main caregiver for an enrolled patient — meaning they are primarily involved in providing care, arranging care, or making decisions about the patient's care, without being paid to do so
- People who are able to speak and read English, Mandarin, or Malay
- People who have access to a phone and/or an internet-connected device
- People who are willing to have their conversations audio-recorded
Who may not be able to join:
For patients:
- People who have cognitive impairment or a serious mental illness
- People who have already had a formal serious illness conversation previously
- People who are not easily reachable by mobile phone or landline
For caregivers:
- People who are employed as foreign domestic workers or live-in helpers
- People who are not easily reachable by mobile phone or landline
