Who might be able to join this trial:
- You are 18 years old or older
- You are receiving care at the outpatient palliative care clinic at Geneva University Hospitals (HUG) in Switzerland
- You experience breathlessness at rest that you would rate between 3 and 7 out of 10 (on a scale where 0 is no breathlessness and 10 is the worst imaginable)
- Your breathlessness has lasted for more than 3 weeks and has not improved enough despite receiving the best available medication for your condition
- You have been in a stable condition, meaning you have not had a serious heart, breathing, or neurological emergency requiring hospitalisation in the past 4 weeks
- You have been diagnosed with one of the following: advanced heart failure (NYHA stage III or IV), severe COPD with significant breathlessness, or a cancer that affects the lungs either directly or indirectly
Who may not be able to join:
- Your breathlessness at rest is rated 8 or higher out of 10
- You have a medical reason that prevents you from having an fMRI scan (confirm with trial site)
- You are unable to lie flat on your back
- You have been diagnosed with a serious psychiatric illness (such as severe depression, severe anxiety, or psychosis), or you take antipsychotic medication that may be a concern for fMRI (confirm with trial site)
- You have a neurological condition, such as dementia, Alzheimer's disease, a brain tumour, a history of stroke, Parkinson's disease, or epilepsy
- You have claustrophobia (fear of enclosed spaces), fear of heights, sensitivity to light, severe hearing loss, or severely impaired vision
- You have migraines, light-triggered epilepsy, or vertigo, which may prevent you from using the virtual reality equipment used in this trial
