Who might be able to join this trial:

• People who have been diagnosed with squamous cell carcinoma (a type of cancer) of the larynx (voice box) or hypopharynx (lower throat), confirmed by a laboratory test, who have not yet received any treatment for it
• People whose cancer is classified as Stage III or Stage IV according to standard staging guidelines (AJCC 8th Edition)
• People whose cancer, including any spread to nearby lymph nodes, is considered potentially removable by surgery and treatable with surgery and combined chemotherapy plus radiation
• People whose general health and ability to carry out daily activities falls within an acceptable range for this trial (rated on a standard medical scale called ECOG, scores 0–2)
• People who are sexually active and able to have children must agree to use highly effective contraception before, during, and for one year after treatment, plus an additional barrier method such as a condom; men must also agree not to donate sperm, and women must agree not to donate eggs during this same period
• Women who could become pregnant must not be pregnant or breastfeeding at the time of screening (women who have had a permanent sterilisation procedure or who have been postmenopausal for at least 12 months are not considered at risk of pregnancy)
• People whose blood counts and organ function (including liver and kidney) fall within acceptable ranges as measured by standard blood tests
• People who are able to understand the study and are willing to sign a written consent form

Who may not be able to join:

• People who have previously received treatment for head and neck cancer
• People whose laryngeal or hypopharyngeal cancer cannot be surgically removed
• People whose cancer has spread to distant parts of the body
• People who have an active autoimmune disease (where the immune system attacks the body) that has required systemic (whole-body) treatment within the past three months, or who currently need ongoing steroids or immune-suppressing medications — though some conditions such as vitiligo, Grave's disease, psoriasis not needing systemic treatment, resolved childhood asthma, hypothyroidism, or Sjögren's syndrome may not be disqualifying (confirm with trial site)
• People who have a history of lung inflammation not caused by infection that required steroid treatment, or who currently have active non-infectious lung inflammation, or have a condition called interstitial lung disease
• People with a known allergy or hypersensitivity to any of the trial medications: carboplatin, cisplatin, toripalimab, or paclitaxel
• People who have had another cancer within the past two years that, in the treating doctor's opinion, could affect the trial results or the person's health
• People who have significant nerve damage causing sensory problems in their hands or feet beyond a certain severity level (confirm with trial site)
• People who have any other condition, ongoing treatment, or abnormal test result that the treating doctor believes would interfere with participation or would not be in the person's best interest
• People with active hepatitis B, active hepatitis C, or HIV infection — though people with a history of HIV may still be considered eligible if their immune cell counts are at an acceptable level and they have had no serious infections in the past 12 months (confirm with trial site)
