Who might be able to join this trial:

• Adults aged 18 years or older
• People who are willing and able to sign a written consent form (for those joining the trial going forward)
• People who fall into one of these health categories: confirmed lung cancer (decided by a specialist team); suspected lung cancer (biopsy came back negative or further tests are still pending); pneumonia (confirmed by scan or clinical assessment); non-cancerous airway conditions such as asthma or chronic obstructive pulmonary disease (COPD); other non-cancerous breathing conditions such as interstitial lung disease, bronchiectasis, or breathing pattern disorder; or healthy people with no known breathing or lung condition
• People whose relevant medical records and diagnosis information are available (for those whose past data may be included)
• People who were previously enrolled in the VICTORY or ExPeL studies, as their past data may be eligible for inclusion
• People willing to complete a breath test using a device called Inflammacheck®
• People willing to allow the trial team to access relevant medical records and scan results
• People who can understand and communicate in English, with or without the help of a translator

Who may not be able to join:

• People who have an active respiratory infection (such as tuberculosis or bronchopneumonia) at the time of the breath test — note that pneumonia itself is not an automatic reason for exclusion
• People who have had major chest surgery or part of a lung removed within the past six months
• People who are currently receiving or have recently received chemotherapy or radiotherapy for lung cancer at the time of breath testing (this does not apply to past participants whose data was collected before treatment began)
• People with severe memory or thinking difficulties, significant communication barriers, or any condition that would prevent them from understanding and agreeing to take part
• People who are pregnant or breastfeeding
• People who are actively using cannabis or recreational drugs that are known to affect the chemical makeup of exhaled breath
• People who have smoked tobacco, vaped, or used e-cigarettes within the 12 hours before the breath test
• People who have taken antibiotic tablets or capsules, or who have started a new course of steroid tablets or steroid inhalers, within the two weeks before breath testing — note this does not apply to people with pneumonia, and people with stable long-term conditions such as asthma or COPD who have been on the same inhaler dose for at least four weeks may still be eligible (confirm with trial site)
• People whose safety or the reliability of the study data could be affected by any other condition, as assessed by the trial doctor
• People who are unwilling or unable to give informed consent
• People who are physically or cognitively unable to complete the breath test
• People who are currently taking part in another interventional clinical trial that could affect breath test results
• People in the lung cancer group who received any systemic treatment for lung cancer before the breath test
• People who have had any active cancer in the past 12 months — excluding certain minor skin cancers or a specific type of non-invasive cervical condition — unless they are in the confirmed lung cancer group
• People who have had a bronchoscopy or an invasive breathing procedure within the 48 hours before breath testing
• People with pneumonia who also have lung cancer, another chest cancer, or lung fibrosis at the same time — people with pneumonia alongside asthma or COPD may be considered at the trial doctor's discretion (confirm with trial site)
