Who might be able to join this trial:

• People who have a confirmed diagnosis of cystic fibrosis (CF), based on genetic testing showing two CF-causing gene changes plus symptoms of CF, or at least one CF-causing gene change plus a sweat chloride test result above 60mmol/L plus symptoms of CF.
• People who are currently receiving care at a UK Adult Cystic Fibrosis Centre that is taking part in this study.
• People who have had at least one CF lung flare-up treated with intravenous (IV) antibiotics in the past 12 months, OR people who have been enrolled in a separate study called CF-Tracker within the last 24 months.
• People whose lung flare-ups are likely to be treated with a specific type of antibiotic combination (a beta-lactam or anti-pseudomonal penicillin, combined with tobramycin or colistin), in line with standard CF treatment guidelines.
• People who are able to produce a sputum (mucus/phlegm) sample — either naturally or with assistance — at the first study visit.
• People who are able to understand the participant information provided and are willing to give their consent to take part.

Who may not be able to join:

• People who are not clinically stable at the time of their first study visit — for example, those experiencing new or worsening symptoms, new viral illness, or who are currently taking extra antibiotics or antiviral medicines (people must have finished any such extra treatments at least 4 weeks before the visit).
• People with extensive allergies or intolerances to antibiotics that would prevent them from being treated with standard CF antibiotic regimens.
• People who have an active tuberculosis (TB) infection.
• People with active ABPA (a specific lung condition caused by a fungal reaction) — meaning those currently being treated for it, those treated within the last 4 months, or those considered likely to need treatment within the next 12 months.
• People who are taking long-term oral steroids at a dose equivalent to 10mg or more of prednisolone per day.
• People who are taking any other long-term medication that suppresses the immune system.
• People who have a non-tuberculosis mycobacteria (NTM) lung infection and are currently undergoing treatment to clear it (people with a chronic NTM infection who are not on clearing treatment and do not expect to start within 12 months are not excluded).
• People who have another condition or health factor that, in the opinion of the study doctor, would make completing the study unsuitable or unsafe (confirm with trial site).
• People who are planning to take part in another clinical trial of a new experimental medicine within the next 12 months.
