Who might be able to join this trial:
- Adults aged 18 or older who are patients at UPMC Clinic and have chronic kidney disease (CKD) that does not require dialysis, shown by a specific kidney function test result, along with at least one of the following: poorly controlled diabetes, high blood pressure over a 12-month average, being overweight or obese, or a positive screening result for difficulty affording or accessing food.
- People who, when assessed by the research coordinator, show some level of readiness to make lifestyle or health changes — including those who are thinking about it, getting ready, already making changes, or maintaining changes they have made.
Who may not be able to join:
- People who are currently on dialysis, or who are expected to start dialysis or receive a kidney transplant within the next 6 months.
- People who show no readiness or intention to make any changes to their health behaviours at the time of screening, as this could affect the trial's ability to measure its main goals.
