Who might be able to join this trial:
- People aged 18 or older who have ongoing lower back pain, have sought or seriously thought about seeking care from a doctor or specialist for it in the past 6 weeks, and find it at least moderately interferes with work or daily activities (including housework)
- OR people aged 45 or older who have hip or knee joint pain that comes on with activity, with no morning stiffness in those joints, or morning stiffness that lasts 30 minutes or less
- People whose average pain over the past week has been rated at least 4 out of 10 on a pain scale (where 0 means no pain and 10 means the worst pain possible)
- People who score 11 or higher on a standard sleep difficulty questionnaire called the Insomnia Severity Index
- People who have access to a computer, laptop, or tablet with internet, suitable for video calls
- People who are willing and able to take part in video consultations
- For people being recruited in the United States: people who are covered under the Military Health System
Who may not be able to join:
- People who are already doing more than 120 minutes per week of at least moderate-intensity physical activity and have been doing so for the past 6 months
- People who are currently taking part in, or have taken part in, a specific type of sleep therapy called CBT-I (Cognitive Behavioural Therapy for Insomnia) in the past 6 months
- People who show signs of significant cognitive (thinking or memory) difficulties, based on a standard screening test (confirm with trial site)
- People who have had an injection of any substance into the area of pain being studied in the past 3 months
- People who are waiting for, or planning to have, an injection into the area of pain being studied within the next 12 months
- People who have had surgery in the past 12 months, or who are waiting for or planning surgery in the next 12 months
- People who have a neurological or whole-body condition that may affect physical function, such as Parkinson's disease, active cancer, multiple sclerosis, rheumatoid arthritis, or ankylosing spondylitis
- People who do not pass a standard pre-exercise health screening questionnaire without first getting medical clearance to participate
- People who are unable to give informed consent or take part in the study's procedures
- People who are unable to speak or read English
