Who might be able to join this trial:

• People aged 15 years or older.
• People who have been diagnosed with a type of acute leukemia (including AML, ALL, or MPAL) based on World Health Organization 2022 guidelines.
• People whose leukemia has a specific genetic feature — either an NPM1 gene mutation (without certain FLT3 mutations), a KMT2A gene rearrangement or KMT2A-PTD, a NUP98 gene rearrangement, or another leukemia subtype that depends on the menin-KMT2A interaction (confirm with trial site for this last category).
• People who have already had a bone marrow/stem cell transplant from a donor, with at least 30 days having passed since the transplant.
• People who have received a type of maintenance medication called a menin inhibitor after their transplant, for at least 2 full cycles (7 days each) or a total of at least 14 days, and who were in complete remission when that medication was started.
• People who have shown no signs of leukemia returning while taking the menin inhibitor — meaning bone marrow blast levels are below 5% and there is no leukemia found outside the bone marrow.
• People whose menin inhibitor was one of the following: Revumenib, BN104, Ziftomenib, HMPL-506, or a similar medication in this drug class.
• People who are able to understand and willingly sign a consent form.
• People whose medical records are sufficiently complete for review.

Who may not be able to join:

• People who had signs of leukemia returning in their bone marrow or blood at the time their menin inhibitor maintenance treatment began (or within 28 days before starting it).
• People who have an active infection that the treating doctor considers uncontrolled.
• People with severe liver problems, as measured by certain blood test results being significantly above normal levels (confirm with trial site for specific thresholds).
• People with severe kidney problems, where kidney filtering function falls below a certain level (confirm with trial site for specific threshold).
• People with serious heart failure classified as NYHA Class III or IV.
• People experiencing a moderate-to-severe immune reaction to their transplant (graft-versus-host disease) that requires high-dose steroid treatment and three or more types of immune-suppressing medications.
• People who have another cancer that is still being actively treated (exceptions may apply for cancers that have been fully treated and are in complete remission with no ongoing therapy needed).
• People with a digestive condition that could affect how oral medications are swallowed or absorbed, such as difficulty swallowing, stomach emptying problems, ongoing severe diarrhea, or gut graft-versus-host disease.
• People taking other maintenance therapies such as hypomethylating agents, donor lymphocyte infusion, or other targeted small-molecule drugs.
• People whose medical records are too incomplete to allow a proper assessment.
• People the treating doctor considers unsuitable for the trial for other reasons.
